New Patients
Welcome to Woods Family Medicine! We understand that choosing a physician can be a process, so we want to give you as much information upfront as possible. Please look through our website, bios, and frequently asked questions (FAQs) pages for more information as well.
What can I expect at my first visit?
Upon arrival, you will be greeted by our clinic coordinator, Teresa. She will help ensure you have completed the new patient forms and that we have the needed copies of your insurance card and driver’s license for verification. Once your insurance is verified, if there is a copayment, Teresa will collect it at this time. You may pay with cash, check, or credit.
The first visit with a physician is a longer visit. We are getting to know your history and learn how we can best help with your medical care. This visit will consist of many questions, several you will have answered many times before for other providers. While this visit can be annoying for some, once all your information is in our system, future visits will be more focused. Unless you have no medical problems, the first visit is never a physical or wellness visit.
What should I bring?
- Recent labs (within the last year)
- Imaging (most recent reports: colonoscopies, mammograms, chest x-rays, etc)
- List of all physicians you have seen in the last 5 years.
- We need specialists’ names and phone numbers so we can request records. There is a new patient form you can download and print prior to your visit to help you gather this information. We will gladly request prior records from your physicians if we have all their contact information and your permission.
- BOTTLES for all the medications that you are currently or should be currently taking.
- (Do NOT bring controlled substances to the office. We can verify recent prescriptions for those online.)
- Insurance Card
- Driver’s license or photo ID
- New patient information form
Do you do lab work in the office?
We do perform some lab work in the office such as strep tests, flu tests, urinalyses, etc., but we do not perform routine blood work in our office currently. We do provide the needed lab requisition forms to take to the lab of your choice for your insurance.
